Carbon Health has partnered with Versima to provide copies of your medical records. If you would like to submit a health record request online, please click one of the links below to submit an authorization form using Verisma's system.

Please note the different authorization forms for requests by patients vs patient representatives. Medical records requests that meet all HIPAA and state requirements are usually available for release within three to five business days.
