We require a valid credit or debit card on file for all appointments. Payment for your initial visit will depend on whether you pay a flat fee or use insurance, and will be due at the beginning of your appointment. After your visit, your bill will be adjusted based on the total cost of the treatments or services you received and your insurance benefits (if relevant).
Yes. Any person who cancels within a 24 hour period (or does not show up to their scheduled appointment time) is subject to a $50 fee. Our clinicians need to balance both scheduled appointments and walk-ins to accommodate as many patients as possible each day.
If you have a legitimate reason why you needed to cancel your appointment in last 24 hours, please call your local Carbon Health clinic to reschedule your appointment.
I got an earlier appointment at another Carbon Health clinic. Will I still be charged a cancellation fee?
Our front desk staff will be able to reschedule your appointment to the preferred location in order to avoid the fee.
There may be times when your insurance plan can’t be verified before your appointment. For example, your insurance information may be inaccurate or out of date, or the insurance company’s eligibility system may not respond. In cases where we are unable to verify your insurance before your visit, we will charge you the flat rate for your treatment or service. These rates are standard and vary by treatment type.
