Covid Travel Clearance Testing

Carbon Health – State & Federal ID Numbers

Carbon Health is authorized to operate a clinical laboratory at all Carbon Health clinics by Centers of Medicare and Medicaid Services (CMS) through Clinical Laboratory Improvement Amendments (CLIA) which is equivalent to ISO 15189 accreditation. https://www.cdph.ca.gov/Programs/OSPHLD/LFS/Pages/CLIA.aspx (opens in a new tab)

ISO 15189 accreditation is an international standard that specifies the quality management system requirements particular to medical laboratories.

If you've tested at one of our main travel clearance sites, please look below for our state and federal CLIA ID number. If you need a copy of the CLIA certificate, you can either visit the clinic where you tested or contact central support to have the certificate sent to your Carbon Health account via the app.

The ID numbers below are for the ** SEND-OUT TEST (RT-PCR) **. For our Rapid test (NAAT) ID numbers, please contact central support.

We cannot guarantee that we meet specific regulations for any other domestic or international travel. We do not provide any clinical sign-offs or wet signatures for other travel forms. It is up to each patient to ensure our testing results and turnaround times are valid for international travel clearance and that any paperwork is handled separately. :::

Carbon Health meets specific guidelines and provides the necessary lab results for the following destinations:

We can't guarantee that we meet specific regulations for any other domestic or international travel. We do not provide any clinical sign offs or wet signature for other travel forms. It is up to the patient to ensure our testing results and turnaround times are valid for their destination and that any paperwork is handled separately.

If you are traveling international, most countries require a COVID-19 RT-PCR and IgM antibody test result. The test must be taken within 72 hours before the scheduled time of departure of the last direct flight to your destination. In some cases the CLIA certificate may also be required. Carbon Health's rapid testing is NOT a RT-PCR test. If you need a RT-PCR test you MUST schedule a send out test where the results are given by 2pm NEXT day.
