What happens if my insurance is unverified before my appointment?
There may be times when your insurance plan can’t be verified before your appointment. For example, your insurance information may be inaccurate or out of date, or the insurance company’s eligibility system may not respond. In cases where we are unable to verify your insurance before your visit, we will charge you the flat rate for your treatment or service. These rates are standard and vary by treatment type. You can find them https://carbonhealth.com/insurance-pricing. As a courtesy, we will also issue a claim to your insurance company on your behalf after your visit. Please check the Carbon Health app for a status on your bill, which may be adjusted based on total treatment costs and your insurance benefits.
I got an earlier appointment at another Carbon Health clinic. Will I still be charged a cancellation fee?
I have a legitimate reason why I needed to cancel my appointment within 24 hours. How can I avoid or reduce the fee?

Do I need a credit card to book an appointment?
We require a valid credit or debit card on file for all appointments. Payment for your initial visit will depend on whether you pay a flat fee or use insurance, and will be due at the beginning of your appointment. After your visit, your bill will be adjusted based on the total cost of the treatments or services you received and your insurance benefits (if relevant).

How do I schedule a COVID-19 vaccine appointment?
Carbon Health no longer offers a general vaccination program to the public, however select locations may be able to provide a COVID vaccine depending on supply availability during an annual check-up. What if there are no appointments available?
If there are no appointments available, you may do a walk-in at our urgent care clinics only. However, please contact the clinic prior to coming in as our normal wait time for walk-ins is 1-2 hours and once a clinic reaches a certain capacity, they’ll stop taking walk-ins. Another option is to schedule a virtual care appointment.

What do I need to bring to my appointment?
If this is your first appointment at Carbon Health, you’ll need to bring the following:
- Government-issued ID (ex. California ID or driver's license or passport)
- Health insurance card
- Valid credit or debit card
If your appointment was made by one of our Central Support Specialists, please make sure to login to your account before your appointment to complete the appointment registration, including:
- Medical history questionnaire
- Insurance or payment information

I'm running late to my appointment, can I still be seen?
Urgent care appointments have a 15 minute grace period, and primary care appointments have a 10 minute grace period. For urgent care appointments, the clinic may be able to take you as a walk-in appointment (with a potential wait time). For primary care appointments, our team will need to reschedule you to the next available date/time. Please note there is a $50 fee for no-shows or cancellations with less than 24 hours notice.
Do you offer virtual care appointments?
Carbon Health offers virtual services for primary, urgent, pediatrics, and mental health care. You can book an appointment by downloading the Carbon Health patient app or going to our website at www.carbonhealth.com.

Do I need an appointment to get a COVID Test?
It’s recommended that you schedule an appointment for a COVID test, which you can do by either downloading the Carbon Health patient app or going to our website at www.carbonhealth.com. If you need a travel clearance test, you’ll need to schedule an appointment at one of our primary care clinics that offers those tests. Select urgent care clinics do accept walk-ins for travel clearance COVID tests, but availability will be limited based on the clinic's current capacity and supplies available.
