First Dose: Before January 10th
- You can expect a communication from Carbon Health (sms or email) between Saturday, January 30th and Tuesday February 2nd, with your appointment details and a rescheduling link.
- Your second dose appointment may be at a different location, and we recommend you keep your auto scheduled appointment. However if you need to reschedule or change location, we recommend doing it immediately as appointment availability is limited.
- Individuals vaccinated between December 30 and January 9 who have not received an SMS or email notification from Carbon Health by February 1:
- Go to Dodger Stadium between 10am - 6pm on the second dose date on their CDC vaccination card or any other day this week to ensure that they receive their second dose within the recommended CDC timeframe.
- You should arrive with their CDC vaccine card, healthcare worker badge, and photo ID. Staff will verify your first vaccination, register you into the Carbon Health system, and administer your second dose.
- If you do not have a vehicle can go City vaccination sites at Lincoln Park (3501 Valley Blvd., Los Angeles, CA 90031), San Fernando Park (208 Park Ave., San Fernando, 91340)
- PLEASE NOTE- walk ins are ONLY accepted if you were originally vaccinated at one of our sites and are served on a first come first serve basis if we have availability. Availability CHANGES everyday depending on no shows and original supply.
- Due to supply uncertainty and site closures due to weather and holidays, appointments may be scheduled earlier or further out, but within the CDC recommended window of 24 to 42 days for Moderna. First Dose: On or After January 10th
- If you received your vaccination at a City of Los Angeles site on January 10th or after, you can expect an email and SMS from Carbon Health 2-3 days before your second dose appointment. This message will include appointment details and a rescheduling link.
- Most of these appointments will be booked at the same location about 28 days after your first dose appointment.
- Due to supply uncertainty and site closures due to weather and holidays, appointments may be scheduled further out, but within the CDC recommended window of 28 to 42 days.
