For your second dose appointment you will receive an email from Carbon Health containing detailed information of your auto-booked appointment.
Please login to your carbon health account to ensure that your contact information is correct and that your first dose appointment is documented in your account. If you do not see your first dose appointment in your chart, you MUST upload your vaccination card.
If you did not receive the email notification, please send us a message via carbon health app informing us that you would like to schedule your second dose appointment. To quickly process your request, again you MUST upload your vaccination card to your account showing proof that you have received your first dose with Carbon Health.
* Please note due to supply uncertainty and site closures due to weather and holidays, appointments may be delayed or scheduled further out, but within the CDC recommended window of 28 to 42 days.
