Carbon Health has launched a digital Health Pass, the first HIPAA-compliant digital vaccine card for patients to use anytime they need proof of their COVID-19 vaccination. As part of the Carbon Health Vaccinate platform, the Health Pass is accessible to all patients who have been vaccinated at a Carbon Health-powered site, or have received a prior vaccination from the Carbon Health network of private and municipal partners.
Once a patient receives their first or second dose of the COVID-19 vaccine at a Carbon Health-powered vaccination site, they will receive a discharge notification via SMS on their cell phone containing a link to their digital Health Pass. Patients can currently access their Health Pass on Carbon Health’s patient desktop web application, and in the Carbon Health mobile apps for iOS and Android.
The Health Pass captures and displays details for first and second dose COVID-19 vaccinations, including dates shots are received, vaccine type and lot numbers, vaccine site information and the administering provider.
To ensure accuracy of the information on your health pass, please login to your Carbon Health account to confirm that your vaccination information is correct. If is not correct, please upload your vaccination card to your account and notify Carbon Health immediately by sending a message via Carbon Health app.

Click the green sign up button
Type in your phone number given onsite
Enter the code texted to you
Input your DOB
Add in your email and create a password
Add in your insurance if you have it, if not choose sign up without insurance
Enter in your address (if prompted)

How can I view my Health Pass?
The electronic health pass is available when you log into your patient account online at patient.carbonhealth.com. Once you log into patient.carbonhealth.com there will be a section on the home screen that says "Your Health Pass is ready. View Card". If you are currently in the downloaded mobile IOS or Android app, you will be able to view it there.
How can I add my Health Pass to my Apple Wallet?
Login to your carbon health account via app and click on "Add to Apple Wallet". You will see a prompt asking you if you want to add to Apple Wallet. Click Add and view in apple wallet. The health pass will display 1 of the 3 Report Types; "Verified" (which means vaccination was administered by Carbon Health), "Self Reported" (patient self reported vaccination) and "Blend" (one dose was self reported and one was verified).
I need my vaccines added to Health Pass that were received at an LAFD location before Carbon Health platform took over for Los Angeles (typically before Feb.1)
The LA County site you were vaccinated at was only using Carbon Health as a scheduling platform at this time. We do not have access to the records in their database to pull from yet. However, we are actively working on this. You’re more than welcome to keep a photo of your vaccination card in our system to store in case you misplace your physical card. For now, you will need to save your vaccination card received at the site you went to as your proof of record.
Can you add my vaccination record to my chart/Health Pass? (Did NOT get vaccinated with LAFD/Carbon)
Being that you did not receive your vaccination through us, we will not have access to the records database to pull it from another vaccination site. We are currently working on this issue and hope to have your data in the coming weeks. I need vaccines added to Health Pass that WERE received at a Carbon/LAFD site / Vaccine card info is incorrect
Please contact support and upload photo (Add file) of your CDC vaccine card and a screenshot of your health pass via carbon health app so that we can have a copy of the information for review. We can then notify our vaccine product team that your health pass needs to be updated. We are working on these updates as quickly as possible and thank you for your patience!
My first and second dose dates are reversed.
We are aware of this issue in the system and will be correcting this within the next week. However, your dates should be correct in the appointment folder of your Carbon Health account.
I lost my vaccination card. Can you provide another one to me?
The California Department of Public Health recommends contacting your local public health department to ask for the records. You can find the phone number for your county online.
